LMAP's Children's programs - Registration Form

Thank you for registering your child/ren for one or more of our children's programs at our Church

We hope to serve both you and your child/ren through our children's programs. We are looking forward to helping your child grow as a disciple of Christ!

Thank you for giving us the details below so we can care well for your child/ren. The information you provide on this form will be considered to be in perpetuity. Please ensure you advise our church office if any information changes.

Please note that all leaders have undergone Safe Ministry Training and undergone a Safe Ministry Check in accordance with the Sydney Diocese Professional Standards, and have a cleared and current Working With Children Check.

We welcome all children to our programs. If your child has extra needs please speak to one of our Ministry Team either in person or via the church office so we can discuss how best to support those needs.

In the case of a medical emergency, I give permission for the leader or other persons supervising or administering the children's program to secure proper treatment for and/or hospitalisation, injection, anaesthetic or surgery for my child as named. I also understand that, if applicable to my child's medical needs, I will need to provide an Epipen for them in the case of an emergency. *

The Lower Mountains Anglican Parish is exempt from the requirements of the Privacy Act 1988 (Cth) as it is a small business. However, we seek to deal carefully with the personal information we collect and to do so in a manner consistent with your reasonable expectations.

By submitting this form you acknowledge that the personal information in this form will be made available to:

(a) the LMAP leaders involved in the running of the activities in which my child participates,

(b) Lower Mountains Anglican Parish's Staff, and

Authorisations & Expectations

I give permission for my child to attend all scheduled LMAP Children's programs I marked above, unless I advise the LMAP Children's program leaders otherwise.
I authorise the LMAP Children's leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.
I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour may result in me and my child being sent home and being temporarily or permanently prohibited from attending the activities of LMAP Children's programs.
I will provide the LMAP Children's leaders with any information relevant to the well-being of my child prior to him or her attending an LMAP Children's program. This might include supporting documentation (eg: AVO, Family Court Order) if there is anyone who is legally restricted from seeing your child.
I give permission for photos/videos of my child taken at LMAP Children's events to be displayed publicly (online and in print) unless I advise the LMAP Children's program leaders otherwise.
I give permission for any online video meetings run by LMAP Children's programs to be recorded for internal compliance purposes.
I confirm that the information given in this form is true and correct, and will advise LMAP Children's leaders of any changes to this information.
