(a) the Youth leaders involved in the running of  Youth Camp,

(b) Lower Mountains Anglican Church's Staff, and

(c) medical and emergency services if considered necessary.

• I give permission for my youth to attend Youth Camp, unless I advise the Lower Mountains Anglican Church Youth Leaders otherwise. 
• I authorise the Youth Leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my youth. 
• I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour may result in my youth being sent home.
• I will provide the Youth Leaders with any information relevant to the wellbeing of my youth prior to him or her attending Youth Camp. This might include supporting documentation (eg: AVO, Family Court Order) if there is anyone who is legally restricted from seeing your youth, or anaphylaxis/allergy plans, and any medication they are required to take.
• I give permission for photos/videos of my youth taken at Youth Camp to be displayed publicly (online and in print) unless I advise the Youth Leaders otherwise. 
• I confirm that the information given in this form is true and correct, and will advise Lower Mountains Anglican Church of any changes to this information.
