Who: Holiday Club is for children in years K to 6.

Date: 14th to 17th of July

Time: 9:30am to 12:30pm (arrive from 9am for check-in)

Cost: $12 per day or $40 for four days

Location: LMAP Glenbrook, 1 Wascoe St, Glenbrook 2773

What: Four mornings of awesome games, amazing craft and age-appropriate teaching about God's story from the bible through drama and song!

Complimentary cafe for parents and carers

Morning tea will be provided for children.

Note: 

LMAP Holiday Club caters for children in years K-6 and is not able to provide care for preschool aged children. Please do not register preschoolers. All leaders have completed mandatory child protection training and have current Working With Children Checks (WWCC).

If your child uses self-calming "tools" (eg noise-cancelling headphones, squishy toy) please bring these to Holiday Club. Your child may use these during big group time, small group time, game times, etc.

Children are put into small groups for some of the activities at Holiday Club. These groups are either K-2 groups or 3-6 groups. Please suggest a friend (or two) your child would like to be in a small group with so that we can make their time at Holiday Club as comfortable as possible.

The Lower Mountains Anglican Parish is exempt from the requirements of the Privacy Act 1988 (Cth) as it is a small business.  However, we seek to deal carefully with the personal information we collect and to do so in a manner consistent with your reasonable expectations.

By submitting this form you acknowledge that the personal information in this form will be made available to:

(a) the LMAP leaders involved in the running of the activities in which your child participates,

(b) Lower Mountains Anglican Parish's Staff, and

(c) medical and emergency services if considered necessary.

• Willing to permit my child to participate fully in the Holiday Club of the Lower Mountains Anglican Church.
• Aware that photos of my child may be taken at the Lower Mountains Anglican Church's Holiday Club and may be displayed publicly (online and in print) unless I advise the leaders otherwise.
• Giving my permission that in the case of a medical emergency, to the doctor chosen (either by the church authorities or other persons supervising or administering the activities), to secure proper treatment for and/or order hospitalisation, injection, anaesthetic or surgery for my child as named. I understand that every effort will be made to contact me prior to instituting such procedures.
